Physician-prescribed, MiCare Path serves as the vital link between specialty care patients and their medical support team outside office visits. Providers track patient progress through data collected by medical devices like activity trackers, thermometers, blood pressure cuffs, and weight scales, which are transmitted through the app. MiCare Path seamlessly integrates with personal activity trackers and Apple HealthKit. The app generates daily analytics to be shared with care teams through a tailored dashboard. Patients can communicate directly with their care teams using the messaging feature. Additional features include access to a personalized education library, the ability to monitor pain and wellness scores, and personalized healthcare plans guidance. CONNECTED DEVICES

MiCare Path is an mHealth remote patient monitoring application prescribed by participating specialty healthcare providers and clinics. The app is compatible with data from approved cellular and Bluetooth devices, as well as Apple HealthKit.

Your provider may request connection of various devices such as an activity tracker, blood pressure cuff, or weight scale depending on your personalized care path. Regular reminders will ensure you use these devices as advised by your physician.

PATIENT CARE PATH

Your individual care plan will be determined by your physician and may include tasks like daily temperature checks, blood pressure monitoring, daily weight tracking, and activity monitoring through your device or wearable. For Bluetooth devices, syncing is required each time you use the device. With cellular devices, information is transmitted instantaneously. Your provider may assign surveys and educational material through the app for real-time sharing and review by your care team.

VITAL RESULTS

Your vital signs data will be shared according to the frequency set by your physician via a personalized dashboard accessible under your profile section on the app.

MESSAGING YOUR CARE TEAM

You can securely communicate with your healthcare provider through the messaging feature within the app.

REQUESTING APPOINTMENTS

Continue scheduling appointments directly with your healthcare provider unless advised otherwise during your discussions within the app's communication platform.
